    Sample HIPAA Certification

    Your certification must be within the last 12 months prior to your rotation start date and must remain valid during your rotation dates. HIPAA certification is valid for 1 year. A completion date on the certificate is required. 

    HIPAA certifications are valid for one year.

    For example, if your rotation starts August 1, 2026, your certification must be valid prior to August 1. 

    In this example, the certification was completed on January 27, 2023 so it is considered valid for a rotation start date of Aug. 1, 2023.

    Take a screenshot of the results and upload them along with your background check. BOTH the OIG check AND the national criminal background check are required. Both must be completed within the last 12 months. You can combine them into 1 PDF (background check + OIG check). Or you can upload the OIG Check separately under Supplemental item in the VSLO portal.

    The University of Utah requires that your drug screen test for at least 10 different substances, must be completed within the last 12 months prior to your rotation dates, and must be valid during the duration of your rotation dates. 

    Please upload the Drug Screen report, that lists the tested substances, to VSLO. Please see example to the right.

    The University of Utah also offers a 10-panel drug screen through the HR storefront for $44.00 or a combined background check and 10 panel drug screen for $68.00.

     

    If you have purchased a 10-panel drug screen through The University of Utah (Certiphi) and have not received an email with further instructions, please contact the Visiting Students team at VisitingStudents@hsc.utah.edu.

    Example of National Criminal Background Check

    Your national criminal background check should have been completed within the last 12 months prior to your rotation start date and must remain valid during the duration of your rotation dates. Background checks are usually valid for 1 year. 

     

    Your background check may include other searches (e.g., county databases, SSN confirmation), but these are not required. We only require that the search was conducted for a national criminal check. 

     

    We have seen students successfully submit national criminal background checks conducted by Certiphi, SentryLink, OneSource, and Checkr among others. There are a number of providers and you are not limited to any specific one. What is more important is that the national criminal background check was conducted in the last 12 months and will remain valid during the dates of your rotation. Please see example to the right.

    The University of Utah also offers a background check through the HR storefront for $24.00 or a combined background check and 10 panel drug screen for $68.00.

    FAQs

    • All of the SFESOM is only offering those electives currently listed in the 2024-2025 Course Catalog in VSLO. Course coordinators are always your best resource regarding the course, scheduling, logistics and opportunities for visiting students. You are welcome to contact course coordinators. You will find them noted in the courses’ listing in VSLO’s 2024-2025 Course Catalog.

    • The SFESOM requires students to apply for our electives through VSLO. Students also need to be in the clinical phase of their training who have completed six of the CORE Clerkships—Internal Medicine, Surgery, Pediatrics, Obstetrics/Gynecology, Psychiatry, Family Medicine and/or Neurology—prior to the requested rotation’s start date to be eligible to participate in visiting rotations at the SFESOM. An affiliation agreement is not required for students to participate in SFESOM electives.

    • Congratulations on receiving an offer to participate in a SFESOM elective. We will send you a WELCOME EMAIL approximately 40 days before your rotation’s start date and the message will include a specific due date to submit your Post-Decision Documents via VSLO. Please be aware students are not required to submit Post-Decision Documentation—nor will the documents be reviewed—until approximately 30 days prior to the rotation start date. 

      Students will need to provide the following as part of their Post-Decision Documentation: 

      1. Criminal Background Check dated within 12 months prior to the rotation’s start date 
      2. Drug Screen dated within 12 months prior to the rotation’s start date 
      3. AAMC Standardized Immunization Form 
      • two doses of Hep B vaccine followed by Hep B antibody testing 
      • QuantiFERON TB Gold or T-Spot dated within 12 months of rotation start date
      • 2024-2025 Influenza Vaccine for rotations beginning on or after September 1, 2024 
    • Re-scheduling in-person rotations are at the sole discretion of the hosting department. Course coordinators are always your best resource regarding the course, scheduling, logistics and opportunities for visiting students. You are welcome to contact course coordinators regarding opportunities to rotate here. You will find them noted in the courses' listing in VSLO's 2024-2025 Course Catalog.

      However, we can reschedule your virtual rotations to a rotation listed in the course catalog if you have a scheduling conflict. Re-scheduling requests for virtual rotations can be submitted via email to visitingstudents@hsc.utah.edu.

    • Extending offers for rotations are at the sole discretion of the hosting department. Course coordinators are always your best resource regarding the course, scheduling, logistics and opportunities for visiting students. You are welcome to contact course coordinators regarding opportunities to rotate here. You will find them noted in the courses' listing in VSLO's 2024-2025 Course Catalog.

    • The Spencer Fox Eccles School of Medicine’s application processing fee is charged for each course regardless if it’s virtual, in-person, 2-week or 4-week. Students are required to pay two fees if they accept two offers to participate in two electives. Also, please remember the fee is non-refundable.
