Federal Application (FAFSA or Renewal FAFSA)
You may apply online at FAFSA. Please complete ALL information on the FAFSA. NOTE: the FAFSA will require 2021 tax information.
When listing the universities you plan to attend, you must indicate where you will be living while attending that university. For example, if you list the University of Utah, indicate where you will live, using the Housing Codes listed on that section of the application.

- The University of Utah and its Title IV Code, which is 003675, must be listed.

School of Medicine Supplemental Application for Scholarships and Loans
In order to be considered for scholarships and institutional loans available through the School of Medicine, admitted medical students must complete the Supplemental Application, which will be available online on November 1st. The deadline to complete this application is March 15th.
Financial aid is not automatically renewed. You must reapply each year.
PLEASE NOTE: All students must be prepared to pay their own tuition and fees by the due date. The Financial Aid Office does not guarantee that your aid will be processed in time to pay tuition by the deadline. However, every effort will be made to help you.

You may be required to submit an Independent Verification form. Only students who are requested by the University Office of Scholarships and Financial Aid to provide additional information need to do so. To avoid delays in processing their application, such students should respond immediately when asked to submit additional documents. Federal funds cannot be disbursed until required verification is completed. Keep copies of your tax forms, W-2's and all schedules.

The University will notify you via U-Mail when you have been offered federal aid. This notice will give you directions to go to your Campus Information System (CIS) website to review your Financial Aid Offer (Award Letter). If you will be accepting a Federal loan, you will need to accept the award via the CIS website.

All students must complete this computer-tutor program once before receiving Direct Unsubsidized and Graduate PLUS. If you have been awarded an Institutional Loan through the School of Medicine, you must complete the Miscellaneous Entrance Counseling each year you receive the loan.
